Once we receive a call for service, we will make an in-depth assessment visit to discuss the client’s needs and to design a care plan specific to the identified needs. We will contact the client’s physician when there is a need for skilled nursing or rehabilitation services to establish an effective care plan. We make every effort to match staff with the client based on the client’s needs. We also assist in finding other community support services.
